TITLE: Health Care Coordinator REPORTS TO: Congregational President FLSA CLASSIFICATION: Exempt, Full Time
SUMMARY: The Health Care Coordinator will serve as an advisor to the Area Delegates of the Congregation in attending to the more complex medical needs of the Sisters in local communities as well as acting as advisor for individual Sisters in managing their medical needs.
ESSENTIAL FUNCTIONS: • Commits to the Mission/Vision of the Sisters of Saint Joseph • Develops a relationship of trust with Sisters throughout the Congregation • Meets with Area Delegates regularly to assess needs and make recommendations regarding a Sister's health issues • Meets with individual Sisters when needed to discuss health issues • Makes hospital visits • Visits Sisters in local communities when recommended by the Area Delegates • Provides consultation with Sisters before and after surgery and medical procedures when requested • When requested, accompanies Sisters with special medical needs to their health care provider • Updates the Congregational President regarding health care concerns of Sister in local communities • Keeps the Director of Admissions at the Villa apprised of potential non-resident admissions • Monitors the progress of non-resident Sisters at the Villa by attending care conferences and Medicare meetings when possible • Assists Sisters in filling out Villa applications, scheduling Villa conversations and coordinating moving to the Villa • Documents important health care information in SSJ FileMaker and Health Care Go on iPad • Assists in the completion of a Sister's Advanced Directive when necessary
EDUCATION: BSN or equivalent
KNOWLEDGE: Medical training and background
QUALIFICATIONS: • Commitment to working together with Area Delegates on behalf of the health of the Sisters of Saint Joseph • Excellent interpersonal communication and collaboration skills • Willingness to work with a mostly geriatric population • Flexibility in order to respond to unexpected medical needs that may arise • Willingness to advocate for a Sister's general health and well-being • Willingness to interface with medical professionals and Area Delegates on issues that are highly confidential and complex in nature • Familiarity with women's health issues • Willingness to travel to carry out responsibilities • Ability to honor confidentiality • Hospital experience
OTHER QUALIFICATIONS: • Proficiency in record-keeping using Microsoft Word, iPad programs and FileMaker Pro • Willingness to become a notary public to assist with the completion of Sisters' Advanced Directives
