CCC offers quality and compassionate human services to all people, especially the most vulnerable, regardless of faith. We are passionate about the work we do in our communities, providing 31 critical services at 11 locations throughout Virginia.

    Summary: This position is responsible for providing post-adoptive case management services to adoptive families. Position available to start on July 7th

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

    Conducts in-person visits to assess stability and family's needs.

    Develops and implements family specific service plans.

    Links the family to services and supports in the community to meet the identified needs.

    Assists the family directly which may include transportation assistance for the purpose of obtaining needed resources, including crisis assistance supports.

    Coordinates services with other agencies and providers.

    Enhances community integration through increased opportunities for community access and involvement.

    Makes collateral contacts with community resources and support services to prevent dissolution of adoptions.

    Monitors service delivery through contacts with service providers as well as contacts with the family.

    Provides post-adoption education and resources that guide families and develop a supportive relationship that meets the identified needs of each family.

    Participates in crisis intervention as needed.

    Maintains case management documentation and complies with grant reporting requirements.

    Completes discharge planning with family to ensure on-going access to services and prevent disruption of services.

    Represents the Agency in the community and workplace in a professional and ethical manner.

    Demonstrates sensitivity to the service population's cultural and socioeconomic characteristics.

    Demonstrates strong interpersonal, decision-making, problem-solving, oral, and written skills.

    Utilizes a basic knowledge of Microsoft Office applications.

    EDUCATION and/or EXPERIENCE:

    A doctorate or master's degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, psychology, special education, or counseling, with a student placement in casework services to children and families or one year of experience in providing casework services to children and families;

    A baccalaureate degree in social work or a field related to social work including, but not limited to, sociology, psychology, special education, or counseling and one year of experience in providing casework services to children and families; or

    A baccalaureate degree in any field plus two years experience in providing casework services to children and families.
