A Samaritan Place Case Manager is responsible for coordinating the case management services provided to non VOCA clients at Samaritan Place and the Children's Emergency Shelter (as needed) in accordance with agency policy and procedures and best practices. Case Managers coordinate program admissions, caseload assignments, and the day-to-day delivery of services. Case Managers ensure clients receive services that are strengths-based, culturally sensitive, and adhere to the house rules. This position has rotational on-call duties, reports to the Lead Case Manager, and has no supervisory responsibilities.
SCHEDULE: 35 hours/week | Monday - Friday | 8:30 AM - 4:30 PM

STARTING HOURLY RATE: $18.50
Job Responsibilities:
• Provide comprehensive case management services to non-VOCA clients, including conducting intake assessments, developing services plans, and providing ongoing support to help clients achieve their goals
• Assess clients through the Coordinated Housing Assessment Match Plan (CHAMP)
• Maintain Homeless Management Information System (HMIS)
• Complete transitional housing contracts
• Monitor client's assigned rooms
• Meet with clients on a regular basis to monitor progress in the program
• Make referrals to appropriate community resources as needed
• Ensure continuity of service and care and timely adjustments to service provisions
• Collaborate with other staff and service providers to coordinate the provision of services, such as food, clothing, medical care, and housing referrals, to ensure clients' basic needs are met
• Be an active participant in meetings involving clients
• Maintain accurate and timely documentation of client services, program outcomes, and compliance with funding requirements
• Establish a healthy rapport with clients in order to create a comfortable and safe environment in which clients feel respected, valued, and heard
• Follow all program policies, procedures, and protocols that support the needs of clients and promote the effectiveness of the program
• Respond to crisis situations, such as mental health emergencies or safety concerns, and providing appropriate interventions and referrals as needed
• Complete documentation and reporting in an accurate and timely manner
• Serve as liaison with community groups and agencies
• Attend weekly case management team meetings
• Be on-call for emergencies or arrange for 24/7 on-call coverage for emergencies
• Other duties as required by the Lead Case Manager and executive staff
Job Requirements:
• Must be able to move objects weighing up to 40 lbs.
• Must hold a valid driver license with an F-endorsement and maintain a clean driving record
• Must be flexible and able to work occasional alternate shifts including weekends, weekdays, and holidays
• Must be willing to travel if required
• Must maintain any training and certification requirements
• Must support the teachings of the Catholic Church and embody our organization's mission and culture through behavior and daily interactions
• Regular and predictable on-site attendance is required
• Must be able to assume rotation of on-call responsibilities for program situations
Job Qualifications:
• Bachelor's degree in Social Service or related field required
• 1 - 3 years of experience in case management is required, preferably in a shelter or housing program
• Excellent communication and interpersonal skills, with the ability to work effectively with clients, staff, and other service providers
• Knowledge of community resources and services, including mental health, medical, and housing resources
• Demonstrate the ability to successfully work independently or as part of a team
• Strong organizational skills and ability to prioritize in order to meet deadlines
• HMIS experience a plus
• Experience with MicroSoft Word, Excel, and Outlook
• Ability to maintain accurate and timely records and reports
• Strong commitment to advocacy for marginalized populations
