The Caritas Program is a parish based social service program. The program provides counseling and case management services to a low income primarily Latino community.
GENERAL JOB DESCRIPTION The case manager is responsible for assessing the needs of clients and assisting them directly or making appropriate referrals. Assistance with charity care applications and housing applications are also part of the responsibilities. Additional responsibilities include, organizing some community events and maintaining community partnerships.
DUTIES AND RESPONSIBILITIES • Help with housing, utility and food assistance • Provide necessary assessments and referrals • Provide linkage to other community agencies for legal, immigration and health services • Assist with Charity Care Applications • Function as a team member communicating all relevant information to other staff members • Attend staff meetings • Some evening and weekend work required
QUALIFICATIONS FOR THE JOB Education: Bachelor's degree required. Experience: Case Management experience preferred. Qualifications: • Bilingual (Spanish/English) • Able to handle confidential personal client information professionally. • Organizational skills with excellent follow up skills a must. • Proficient in Word and Excel
