    Hospice RN Case Manager - Full-Time Monday - Friday

    The RN Case Manager plans, organizes, and directs hospice care provided by the Interdisciplinary Team, with emphasis on community health, education, and experience. The RN Case Manager uses available resources to plan and direct services to meet the needs of patients and families within their homes. EEO.

    Essential Functions:

    Collaborate with physicians and other professionals to plan care.
    Make frequent visits to the patient to evaluate their condition.
    Provide support and comfort according to individual patient's needs.
    Work with caregivers to plan and execute excellent patient care.
    Alleviate sudden symptoms of the disease's end phase.
    Notify a doctor of a patient's condition when deemed necessary.
    Assist patients and their families with paperwork.
    Maintain accurate records.
    Help relatives cope with the upcoming loss and provide post-mortem support.
    Minimum Position Requirements:

    At least one year of current Hospice experience (preferred)
    Graduate of an accredited school of nursing. RN licensure (current, unrestricted).
    Valid driver's license, auto insurance, reliable transportation and willingness to drive to patients place of residence.
    Must have excellent interpersonal skills and a passion for hospice
